SYNOVIAL CYST FACTS
First, let us discuss what synovial cysts are. Synovial cysts are uncommon lesions affiliated with facet degeneration and commonly found in the lumbar spine at L4-S1. (1) 33% to 40% of synovial cysts are linked with degenerative spondylolisthesis and instability of the affected joint. (2,3) Synovial cysts may trigger sciatica/leg pain. (5) When synovial cysts seem lighter on MRI T2 images, they are likely more fluid filled and less likely gelatinous or calcified, making them possibly easier to reduce. (6) Synovial cysts have been reported to disappear particularly if the cyst wall is not calcified. Wilby et al describe a channel that joins the facet joint capsule with the ligamentum flavum, the area where the synovial cysts form.
